  • Not insurance: Membership is not health insurance and does not replace insurance.
  • Membership + insurance model: Membership covers the concierge service model. Insurance may be billed when covered medical services are provided.
  • Minimum term: Initial minimum term is 3 months. After that, membership renews month-to-month unless cancelled with at least 30 days' notice.
  • No enrollment fee: MDHC does not charge a separate enrollment fee for these membership plans.
  • Service area: Home visits are limited to active service areas and provider availability.
  • No visit rollover: Unused included monthly home visits do not roll over to future months.
  • Additional home visits: Members may request additional home visits for $199 when available and clinically appropriate.

Do I need insurance?
