Financial Responsibility Policy

This document is a working operational draft for MDHC. It should be reviewed by qualified healthcare counsel before final publication or use with patients.

Membership Fees

Membership fees are separate from insurance benefits and are not submitted to insurance carriers.

Insurance Billing

When appropriate and permitted by law, MDHC may bill Medicare, Medicare Advantage, Medicaid where applicable, commercial insurance plans, and other third-party payers for covered medical services provided to members.

Patient Responsibility

Members remain responsible for deductibles, co-payments, coinsurance, non-covered services, and services denied by insurance carriers.

Payment Authorization

Members authorize MDHC and its payment processor to charge recurring membership fees using the payment method provided.

Failed Payments

Members are responsible for maintaining valid payment information. Repeated failed payments may result in suspension of membership benefits until payment issues are resolved.

Additional Services

Certain services may generate separate charges, including laboratory services, diagnostic testing, vaccinations, specialty services, and other non-included services. Members will be notified whenever practical before incurring additional charges.

By enrolling or continuing membership, the patient/member acknowledges review of the applicable MDHC policies, agreements, and consents presented during enrollment.
