Membership Cancellation Policy

This document is a working operational draft for MDHC. It should be reviewed by qualified healthcare counsel before final publication or use with patients.

Initial Membership Commitment

All MDHC Concierge Memberships require an initial three (3) month commitment beginning on the membership start date.

Cancellation During Initial Commitment Period

Members may request cancellation at any time. However, membership fees will continue through completion of the initial three-month commitment period.

No separate cancellation fee or early termination fee will be charged.

Cancellation After Initial Commitment Period

Following completion of the initial commitment period, memberships continue on a month-to-month basis. Members may cancel by providing at least thirty (30) days written notice.

Effective Cancellation Date

Membership benefits remain active through the effective cancellation date. Membership fees remain due through the effective cancellation date.

Methods of Cancellation

Cancellation requests may be submitted through the Patient Portal Membership Center, written email request, written mailed request, or other methods approved by MDHC.

Confirmation of Cancellation

MDHC will provide written confirmation of the cancellation request and effective termination date.

Refund Policy

Membership fees are generally non-refundable except where required by law or approved by MDHC management.

By enrolling or continuing membership, the patient/member acknowledges review of the applicable MDHC policies, agreements, and consents presented during enrollment.
