At-Home Primary Care

At-Home Primary Care Visit Membership

In-person visits in the comfort of your home.
$249 / month
or $2,700 / year (prepaid)
Includes up to 2 at-home visits per month within our service area.
  • Comprehensive primary care visits in your home.
  • Telephonic/virtual follow-up between visits as needed.
  • Medication review and care coordination.
  • Additional at-home visit: $125 per visit.
  • One-time onboarding visit and enrollment fee: $99.
  • 3-month minimum term, then month-to-month.
Enroll in At-Home Plan Service available only in designated coverage areas. Visit availability and scheduling windows may vary based on provider availability.
Virtual Primary Care

Virtual Primary Care Visit Membership

Flexible and convenient care by video or phone.
$99 / month
or $1,080 / year (prepaid)
Includes up to 4 virtual visits per month.
  • Video or telephone primary care visits.
  • Prescription management and lab ordering coordination.
  • Portal messaging access for follow-up questions.
  • Additional virtual visit: $35 per visit.
  • One-time virtual onboarding fee: $49.
  • 3-month minimum term, then month-to-month.
Enroll in Virtual Plan Virtual care is provided to patients located in states where our providers are licensed at the time of the visit.

Key terms & conditions (summary)

The following is a summary of important membership terms. The full details are set out in the Membership Subscriber Agreement you will review and sign during enrollment.

  • Not insurance: Membership is not health insurance and does not replace insurance.
  • Billing & payments: Membership fees are billed automatically to the card on file each month (or annually if prepaid).
  • Minimum term: Initial minimum term is 3 months. After that, membership renews month-to-month unless cancelled with at least 30 days’ notice.
  • Service area: At-home visits are limited to our defined service area. If you move outside the area, we may transition you to a virtual-only plan.
  • No visit rollover: Unused included visits do not roll over to future months.
  • Additional services: Services not listed in the plan (e.g., after-hours urgent visits) may be billed separately.
